THIS IS REQUIRED FOR 7th-12th GRADES ONLY!
Attention Parents and Guardians: the OSSAA has partnered with Rank One to help simplify the eligibility process.
All MS and HS Athletes or any student participating in all Sports and Cheer are required to obtain a physical. Parents MUST also create an account on Rank One to complete the required documentation to participate. Please complete these forms at your earliest convenience.
Follow the directions below to complete the process.
1. Go to www.rankone.com
2. Select “Parents click here”
3. Click "Go to Forms" under Online Forms
4. Scroll to select “Oklahoma”
5. Scroll to select “Springer
Public Schools”
6. Select “Create new account”
7. When creating an account, you must know the student ID number. That number can be found on your Sylogist parent portal.
8. Once there, you will fill out the two required forms.
9. Once the forms are complete and correct, your child’s name will turn green in the system. This can take a while to process.
If you need assistance, or the use of a computer, the high school will be open Monday through Friday 8:00 to 3:00
